RESUMO

Objective:To explore the effect of group sandplay therapy on children with autism spectrum disorder(ASD).Methods:Ninety children with ASD of Zhuang nationality were randomly divided into individual sandplay group, parent-child sandplay group and peer sandplay group, with 30 cases in each group. All the children were taught by the method of applied behavior analysis (ABA). Individual sandplay group was added with individual sandplay therapy, parent-child sandplay group was added with parent-child group sandplay therapy, and peer sandplay group was added with peer group sandplay therapy. Autism Behavior Checklist(ABC), Childhood Autism Rating Scale (CARS), Psycho-educational Profile for Autistic and Developmentally Disabled Children Version 3(PEP-3), Social Responsiveness Scale (SRS) and Assessment of Changes in Behavioural Trends were used to evaluate the efficacy before and after treatment for 6 months. SPSS 24.0 software was used for F test, paired t test and nonparametric test for statistical analysis. Results:After intervention, the scores of social interaction(16.90±2.14, 16.56±1.87), sports(20.60±2.43, 20.10±2.25), self-care(16.90±2.14, 16.93±2.30), ABC (97.53±7.18, 96.57±6.61)and CARS(36.10±2.29, 36.10±2.45) in parent-child and peer sandplay group were significantly different from those((19.43±3.22), (22.77±2.24), (19.87±3.60), (108.23±7.59), (37.80±2.92)) in individual sandplay group ( P<0.05). There were significant differences in the efficacy of imitation (76.67%, 80.00%), social interaction (83.33%, 86.67%) and emotional expression (80.00%, 83.36%) between parent-child and peer sandplay group and individual sandplay group ( P<0.05). The scores of social cognition(16.07±3.20, 17.07±2.26), social communication(31.43±3.84, 33.20±3.64), autism behavior(23.90±3.60, 25.93±4.57)and SRS(101.97±9.81, 107.57±8.20)in parent-child and peer sandplay group were significantly different from those((18.17±3.05), (36.53±4.03), (27.90±5.22), (117.07±10.09))in individual sandplay group ( P<0.05). There were significant differences in the number of eye contact((2.26±0.78)times/unit, (2.33±0.80))times/unit) and physical contact((2.27±0.78))times/unit, (2.33±0.80))times/unit) between parent-child and peer sandplay group and the number((1.70±0.79))times/unit, (1.83±0.77))times/unit) of individual sandplay group( P<0.05). Conclusions:Group sandplay can significantly improve the social interaction of Zhuang children with autism spectrum disorder than individual sandplay. However, peer group sandplay has the same effect on language and social motivation, and parent-child group sandplay has the same effect on sensory ability and social perception as individual sandplay.

RESUMO

BACKGROUND/AIMS: In recent years, greater assessment accuracy after transjugular intrahepatic portosystemic shunt (TIPS) to ascertain prognosis has become important in decompensated cirrhosis due to portal hypertension. The aim of this study was to assess the ratio of the portal pressure gradient (PPG) pre-TIPS (pre-PPG) to albumin (PPA), which influence ascites formation in cirrhotic patients in the 6-months after TIPS placement, and is a metric introduced in our study. METHODS: This was a retrospective cohort study of 58 patients with decompensated cirrhosis admitted to an academic hospital for the purpose of TIPS placement. We collected the following data: demographics, laboratory measures, and PPG during the TIPS procedure. Then we analyzed the association between the above data and ascites formation post-TIPS in cirrhosis patients. RESULTS: Twenty-two patients with ascites and 28 without ascites were evaluated. Univariate and binary logistic regression analysis were adjusted for the following variables: to determine prognosis; Child-Pugh scores, lymphocyte count, platelet count, hemoglobin level, albumin level and pre-PPG or PPA. The outcome showed that PPA was better than pre-PPG and albumin for predicting ascites according to area under receiver operating characteristic curves and a statistical model that also showed PPA’s influence 6-months post-TIPS. CONCLUSIONS: The combined measurement of pre-PPG and albumin, defined as PPA, may provide a better way to predict post-TIPS ascites in decompensated cirrhosis, which underlines the need for a large clinical trial in the future.

RESUMO

Pace of life and work of people is accelerating nowadays, and hospitals keep improving their services, which gives rise to the decoction and delivery service from the third party vendors for traditional Chinese medicine(TCM).Given the quality control standards for the TCM decoction service as issued by Zhejiang and other regions, the systems of supervision and assessment remain incomplete.Authors of the paper introduced a project improvement team, composed of Chinese medicine pharmacy, Chinese medicine experience specialists, vendors of Chinese medicine decoction and express delivery companies, hence establishing a " three-in-one" supervision system of Internet+TCM decoction and delivery service. This practice can optimize the assessment indexes, strengthen the assessment system of assessment transformation and supervision system for patient medication. It proves that the practice contributes to higher quality and safety of TCM decoction and delivery service, improves the ability and level of TCM services, and ensures the medication safety of patients.

RESUMO

Objective To investigate the angiographic manifestations of renal artery injury caused by percutaneous nephrolithotomy, and to evaluate the therapeutic effect of super-selective renal arterial embolization in treating renal artery injury. Methods A total of 22 patients with persistent or intermittent gross hematuria that occurred after percutaneous nephrolithotomy, who were encountered at authors’ hospital during the period from Jan. 2010 to June 2014, were included in this study. The diagnosis was confirmed by renal angiography in all patients, and super-selective renal arterial embolization with steel micro-coils was carried out in all patients. The patients were followed up for three months. The results were analyzed. Results Of the 22 patients, DSA examination showed that renal artery pseudoaneurysm (RAP) was found in 14 (63.6%), renal arteriovenous fistula (RAVF) in 5 (22.7%) and RAP associated with RAVF in 3 (13.6%). Renal angiography performed after super-selective renal arterial embolization showed that complete obstruction of the bleeding arteries was achieved in all patients, and the active bleeding stopped. Both the technical success rate and the hemostasis rate were 100%. During the follow-up period lasting for three months, no recurrence of hematuria or severe complications occurred. In 20 patients, different degree of embolism syndrome was observed after the treatment. Conclusion Renal artery pseudoaneurysm and renal arteriovenous fistula are the main types of renal artery injury after percutaneous nephrolithotomy. Super-selective renal arterial embolization with micro-coils can be used as the treatment of choice for patients who has failed to respond to conservative therapy.

RESUMO

OBJECTIVE@#To explore a method of demonstrating the facial nerve anatomical landmarks under transmastoid and epitympanum approach with multi-slice CT using double oblique multi-planar reconstruction (MPR).@*METHOD@#Two temporal bone of a corpse were dissected, under transmastoid and epitympanum approach, to observe the anatomical landmarks of facial nerve. Based on that, the anatomical landmarks of facial nerve under transmastoid and epitympanum approach in 30 (60 ears) normal temporal bones of adult corpses were reconstructed using double oblique MPR in multi-slice CT. The achievement ratio was calculated and the differences among transverse plane, coronal plane, sagittal plane and double oblique were compared.@*RESULT@#The different part of facial nerve, such as mastoid segment, tympanum segment, pyramid segment, geniculate ganglion and the outer labyrinthine segment could be exposed clearly with the main anatomical landmarks, such as horizontal semicircular canal, epitympanic recess and cochleariform process through transmastoid and epitympanum approach. The image of anatomical landmarks could be showed in the same sections by double oblique multi-planar reconstruction. The double oblique multi-planar reconstruction to show the landmarks of facial nerve displaying on the same imaging is better than transverse plane, coronal plane and sagittal plane. The achievement ratio of every section is 100%.@*CONCLUSION@#Double oblique MPR is a new method to demonstrate anatomical landmarks through transmastoid and epitympanum approach in one slice. Combined with the operative approach and purpose, the reconstructive images with double oblique MPR can provide valuable information for operation.
